Through our friends at Evoke Medical, SoZen providers can recommend that certain eligible patients receive a tailored monitoring devices to keep a closer eye on patient's health conditions. These devices are typically covered by insurance and include weight scales, blood pressure cuffs, and glucose meters depending on individual needs.
Evoke medical's team continuously reviews the data and notifies the patient and our healthcare team regarding any out-of-range readings, enabling proactive health management to prevent serious issues before they arise.
